What our packages include
- During the first visit
- Assessment, planning, and coordination of personalized nursing and care services
- Organization of medication regimen and explanation of proper administration according to medical instructions
- Consultation for creating and maintaining a healthy and safe environment
- Liaison and coordination with other healthcare professionals when needed
- At every visit
- Monitoring and evaluation of vital signs (blood pressure, pulse, oxygen saturation, blood sugar, etc.)
- Assistance with personal hygiene (eg. bathing)
- Medication management
- Pressure ulcer prevention
- Caregiver/family training on fall prevention and daily living routines
- Services that may be included based on assessment
- Blood sampling and collection of biological specimens, with transfer to a clinical laboratory
- Administration of injections
- Nasogastric tube replacement
- Placement and replacement of urinary catheter
- Stoma care and changes (tracheostomy, gastrostomy, colostomy, ileostomy)
- Other specialized nursing interventions
- Physiotherapy and rehabilitation
- The planning and supervision of the service will be by the head nurse who will also be responsible for the plan. Depending on the needs of the individual, the service will be performed by trained staff (nurse or caregiver).
- Important notes:
- Each visit will have a maximum duration of up to one hour, depending on the need to implement the personalized care plan
- In case there is a need to extend the duration, this will carry an additional charge and prior consent from the individual and/or family
- The package price does not include consumables which will carry an additional charge. The individual and/or family will be informed by the head nurse about the possible need for consumables, which they may choose to purchase independently if they wish.
